Study in collaboration with Heidelberg University Hospital: GalvoSurge vs AirFlow in surgical peri-implant therapy

The study will be carried out as part of the regular medically indicated peri-implantitis therapy in the rooms of the MKG Surgery Heidelberg or the practice "Bella&Dent Hanau".

What is peri-implantitis?

Peri-implantitis is a chronic inflammation around implants that are firmly anchored in the bone. The inflammation leads to a loss of bone around the implant and, if left untreated, to the loss of the implant. The disease is triggered by bacteria that organize themselves in the form of a biofilm, especially when risk factors such as smoking, diabetes or existing periodontitis are present. The patient often does not notice this inflammation, but occasionally there may be bleeding at the implant or slight pain/swelling.

During dental examinations, however, this inflammation can be clearly detected on the basis of various measured values. The goal of therapy is to remove the biofilm and stop the inflammatory process. In addition to intensified hygiene measures, local rinsing with disinfecting solutions, surgical removal of the inflamed tissue and cleaning of the implant surface is always indicated in cases of pronounced periimplantitis. According to the current guideline, this involves complete removal of the inflamed tissue and cleaning of the implant surface with instruments and a special powder blasting device.

However, the current study situation shows that the long-term success rate of complete inflammation clearance is only 58% despite guideline-compliant treatment.

How does the study work?

The study will be conducted as part of the regular medically indicated peri-implantitis therapy in the rooms of the MKG Surgery Heidelberg or the practice “Bella&Dent Hanau”. It includes the initial findings survey, the surgical procedure itself, and a total of four follow-up visits at 2 and 4 weeks and at 6 and 12 months. The time required and scope of the procedure correspond to normal peri-implantitis therapy.

As this is a “blinded, randomized” study, patients are randomly assigned to either Group 1 (GalvoSurge – electrolytic cleaning procedure) or Group 2 (AirFlow – powder jet device) prior to baseline findings. Study group assignment will be communicated to patients either after completion of data collection or upon specific request.

The latter automatically leads to withdrawal from the study, but has no disadvantages for the patients.

What is the objective of the study?

The objective of this study is to compare the electrolytic cleaning process and a powder blasting device in terms of their effectiveness for cleaning implant surfaces. In the electrolytic cleaning procedure, the exposed implant surfaces are rinsed with an electrolyte solution and a minimal electrical voltage is generated on the implant via two small electrodes. The resulting hydrogen bubbles remove the biofilm. The powder blasting unit operates with compressed air and powder containing glycine. This irradiates the exposed implant surface in a circular pattern and thus cleans it.

The aim of the study is to find out which procedure in the patient’s mouth leads to improved cleaning and long-term elimination of peri-implant inflammation.
